DFD’s nurse care management team is a group of specialized registered nurses who manage patients’ long-term care plans. This team works closely with patients and their loved ones to create a comprehensive health plan that caters to each individual’s unique healthcare needs. A nurse care manager coordinates the care patients receive and acts as an advocate for the patient, assuring the most effective and cost-efficient care possible.
